Glory be to God Ministries is a equipping and prayer ministry. It is not professional counseling. It is always advisable for me to actively pursue spiritual disciplines such as, but not limited to; church attendance, prayer, Bible study, and worship as I actively participate in deepening my relationship with Jesus. By filling out the form below, I understand that my personal files are protected as "CONFIDENTIAL". As such, identifying specific details of my file may not be disclosed to others without my written consent except as specified by Florida law: 1) as required by subpoena or court order in legal proceedings, 2) when not to do so would potentially result in physical harm to myself or others, 3) to report any disclosure\suspicion of child\elderly neglect or abuse (physical or sexual). Also, I further understand that Glory be to God Ministries, Inc.is an equipping and prayer ministry and not a counseling or therapeutic center.
I also understand that this ministry uses the services of volunteer prayer ministers. As such they are required to consult with the Equipping Director/Manager at times.
I give my permission for my prayer minister(s) to discuss my needs with the appropriate leadership as necessary. If I have any questions concerning
my prayer minister’s training, experience or qualifications, I have the right to consult with the leadership of Glory be to God Ministries, Inc.
I understand that this ministry has a limited number of prayer ministers that occasionally must cancel for extenuating circumstances. In the event that my prayer minister must cancel without advance notice, I agree to either reschedule my appointment or take the next available prayer ministry time. I also understand that I am protected by law from any sexual advances or harassment while undergoing ministry. If at any time I feel offended by anything my prayer minister asks, says or does, I acknowledge that is my responsibility to report it immediately to the Equipping Director/Manager. I understand that because Glory be to God Ministries is a prayer ministry and not a counseling or therapeutic center, Glory be to God ministries does not routinely keep detailed records and will not provide information for lawyers, or for disability claims, or for similar matters.
Regarding online appointments, where applicable, I understand that I will be using a 3rd party video conferencing program to communicate during my online prayer appointment. Glory be to God Ministries will not record these ministry appointments and will work to keep my time of ministry as secure as possible. With any online platform, there are certain security risks. In the rare case of a security breach I will not hold Glory be to God Ministries, Inc. liable for any security issues pertaining to my online prayer appointment.
I understand Glory be to God Ministries and its ministers can not control God or promise what He will do. I understand they can only promise that they will do their best to hear from God for my good and God’s glory. Everything shared with me should be taken to those in whom I trust for wisdom, evaluation, and prayer. If I believe what was shared is from the Lord I will pray for it to come to pass. If I and those I sought for wisdom don't believe it is from the Lord, I WILL DISCARD what was shared during my ministry session(s). It may be advisable for me to receive Christian professional counseling as well as prayer. I understand this is strongly encouraged, especially in dealing with dysfunctional habits.
Additionally, I agree to comply with all Glory be to God Ministries, all Federal or State Laws,
municipal or county ordinances, rules, regulations, and codes respecting the premises in which I will meet for prayer with ministers.
For your safety and the safety of others cameras are throughout the buidling hallways and in the prayer room.
I hereby release all Glory be to God Ministries and its ministers from any liability should this ministry session not live up to my expectations or
lead to any spiritual, emotional or functional, physical dysfunction.
